Acceptable forms of Proof of Ownership
Current copy of a phone bill from your provider with the date of Installation or Purchase Order which includes:
Full name
Billing address
Phone number
All documents must be on phone company letterhead.
No handwritten documents will be accepted. Customer will need to include a cover sheet that includes details as to why they are sending in the proof of ownership.
